• As a Hospice RN at Addus Homecare / Serenity Hospice, you will leverage your extensive experience to : *
• Lead Patient Care : * Oversee patient and family caseloads with a focus on comprehensive assessment, care planning, and implementation.
• Coordinate with Teams : * Collaborate with physicians, interdisciplinary teams, and other agencies to ensure seamless, high-quality care.
• Travel with Purpose : * Visit patients within a 30-mile radius, with mileage reimbursement provided.
• Deliver Compassionate Care : * Perform clinical pronouncements of death, support families, and maintain meticulous patient records.
• Your Qualifications : *
• Professional Credentials : * Graduate from an accredited nursing program, hold a current RN license, and possess CPR certification.
• Travel Ready : * Willingness to travel within a 30-mile radius and have a valid driver’s license with proof of insurance.
• Quality Focused : * In-depth knowledge of quality assessment and assurance procedures.
