The LPN / LVN provides professional nursing care to patients within an assigned unit, clinic or region.

The LPN / LVN assesses patient health problems and needs, takes vital signs, administers medication, maintains medical records.

May advise patients on health maintenance and disease prevention or provide case management.

Understands own work area professional concepts / standards, regulations, strategies and operating standards. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization / timing, and works under minimal direction.

Assists the market with compliance and education. Assist Market Preceptor in training new hire Medical Assistants and reinforcing Best Practice protocols.

CenterWell is seeking a dedicated, compassionate, and experienced - LPN / LVN - who is interested in growing with the company, working in multiple locations to support our Middle TN market and being a part of something great!

This is a market float position that is required to support multiple centers across Middle Tennessee(including Clarksville, Lebanon, and Murfreesboro) .

M ileage is reimbursable

LPN Role Overview :

Prepare patients for examinations, including taking vitals

Recording & updating patient medical histories in electronic medical records software

Phone Triage using critical clinical judgement.

Delivering results to patients as providers direct.

Performing in-clinic routine tests, screening and point of care testing such as blood draws, EKGs Orthostatic Blood Pressures, Strep, COVID and FLU Nasal and Oral Tests

Scheduling and assisting with external routine screening and diagnostic tests such as mammograms, colorectal screenings, echocardiograms, and bone density screenings.

Medication adherence and refilling medications

Preparing and administering medications, including intramuscular, intradermal, and subcutaneous injections-including vaccinations / immunizations, as directed by a physician or other licensed provider

Counseling patients on medication and special diets

Wound care and changing dressings

Sterilizing and cleaning equipment and ensuring maintenance of examination rooms

Collaborates closely with physicians and nurses to ensure top level patient care

Highly accurate in collecting phlebotomy samples from senior-aged patients

Required Qualifications

Current licensure as an LPN or LVN

Phlebotomy experience

Reliable transportation

Med adherence / refilling meds

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Excellent customer service

Strong clinical background

The ability to adapt and work in multiple clinical settings

Strong sense of engagement to be a team player

Experience in a fast pace / high volume environment

Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Preferred Qualifications

2 years clinical experience

Hands-on professional

Phlebotomy experience

Experience with Electronic Medical Records

Experience with HEDIS

Knowledge of primary care medicine requirements (state specific).

Working knowledge of clinical operations

LPN / LVN Working hours :

Scheduled 40 hours per week

Monday to Friday 8AM-5PM

Local travel may be required; Mileage is reimbursed

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.

The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
