Family Home Health Network , is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled / Custodial private care services throughout the Chicagoland area.
We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills.
You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility / home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.
Qualifications :
- Must possess current Illinois RN licensure
- Possess a minimum of two (2) years experience (Hospice Preferred)
- Current CPR
- Valid IL driver’s license and auto insurance policy
- Consolo EMR experience a plus
- Must possess excellent clinical nursing and customer service skills
