Ove rview :
The Referral Coordinator provides medical referral management for Fourth Street Clinic patients. The primary duties include supporting the mission of the organization with patient-oriented care coordination.
Referral Coordinators work closely with Medical Providers and Behavioral Health staff to provide a full range of health, medical, and social services to those patients identified as needing specialized and resource-intensive services, medical referrals to community partners, and follow-up with medical care in the clinic.
Duties & Responsibilities :

Core Responsibilities :
- Work closely with the medical team providers, and front desk to provide scheduling support, and look up medical records from outside clinics and hospitals, including patient results, appointments, imaging, and other medical records.
- Provide compassionate and trauma-informed care.
- Schedule both outgoing and internal referrals in the community or at the Fourth Street Clinic.
- Follow up on referrals daily and assist with scheduling needed appointments or closing out referrals.
- Working with team members in all parts of the clinic to ensure the patient is getting thorough care.
- Connecting with patients while they are in-clinic or via phone to assist in coordinating appointments, transportation, or other needs
- Answer phone calls from patients and outside clinics regarding patient appointments and needed documentation.
- Verify insurance through the eligibility lookup tool.
- Refer non-insured patients to community resources for medical appointments.
- Determine patient qualifications and collect documentation for voucher programs, fill out and track all DI vouchers, cab vouchers, and bus passes given to patients.
- Update tracking sheets and patient charts when needed.
- Assist with applications and placement for Skilled Nursing Facilities and Hospice Care.
- Place patients at designated a hotel through our Respite Care Program.
- Acquire durable medical equipment, oxygen, or other needed medical devices for the patient through health insurance or charitable donations.

- Relevant experience with underserved populations
- Communication skills
- Strong interpersonal skills
- Progressive and self-motivated
- Able to multitask
- Able to keep track of workflow
- Ability to be held accountable and take constructive feedback
- Skilled in effective oral communication techniques; able to communicate technical material or highly complex issues
- Ability to write effectively to capture detailed and relevant information regarding patient care referral and follow-up care
- Bilingual in Spanish preferred
- Experience in EPIC or other EMR databases preferred
- College education preferred, at least associate level, but will consider experience in areas of social services, psychology, case management, or direct patient care with underserved or indigent population
