Job Summary :

The position is responsible for promoting the efficient and effective use of a behavioral health care by focusing on the whole patient to provide a seamless continuum of care for Oyate Health Center (OHC) patients.

This position consults with patients and / or their families to facilitate achieving treatment plan goals, developing life skills, and / or sustaining financial stability, conducts activities and monitors the service environment in a manner to maximize patient success and well-being.

Essential Functions :

Facilitate patient access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services;

assist to develop resources and make contact with social support networks.

  • Assist patients in developing goals and areas of need, assists in developing treatment plans which are assessed regularly.
  • Work with agencies and service providers to facilitate case coordination and information sharing.
  • Coordinate and monitor services, including comprehensive tracking of patient activities in relation to care plan.
  • Document all patient encounters; complete and submit billing documentation as appropriate; maintain comprehensive patient files.
  • Identify and provide emergency crisis services as necessary; make immediate assessment and respond according to accepted crisis intervention methods and techniques;

coordinate other services as appropriate.

  • Assess patients, evaluates effectiveness of care plan and progress made by client, participate in patient treatment planning and case review with patient care providers.
  • Promote effective and efficient utilization of clinical resources.
  • Develop effective working relations and cooperate with medical team throughout the entire case management process.
  • Maintains and reports applicable statistics regarding programs and patient services.
  • Must comply with federal laws and regulations as required by the Health Insurance Portability and Accountability Act (HIPAA).
  • Performs miscellaneous job-related duties as assigned.
  • Other duties as assigned by the Supervisor

  • Valid driver’s license must be kept current, and certificates, credentials or licenses must be kept current and consistent with regulations required by applicable federal, state and / or grant regulations.

  • Embrace modes of appearance and attire that reflect a professional presence.
  • Adhere to GPTLHB policies and procedures.

Supervisory Controls

The supervisor provides continuing or individual assignments by indicating generally what is to be done, limitations, quality and quantity expected, deadlines, and priority of assignments.

When there are new, difficult, or unusual assignments, the supervisor provides additional, specific instructions for including suggested work methods or advice on source material available.

The employee uses initiative in carrying out recurring assignments independently without specific instructions, but refers deviations, problems, and unfamiliar situations not covered by instructions to the for decision help.

Guidelines

There are established procedures for doing the work and a number of specific guidelines are available as a resource.

The employee may be required to use judgment in locating and selecting the most appropriate guidelines, references, and procedures for application and in making minor deviations to adapt the guidelines to specific cases.

The employee may also determine which of several established alternatives to use. Situations to which the existing guidelines cannot be applied or significant proposed deviations from the guidelines are referred to the supervisor.

Guidelines include GPTLHB, OHC, and department policies and procedures. These guidelines are generally clear and specific, and deviations must be authorized by the supervisor.

Complexity / Scope of Work

The work consists of related steps, processes, or methods. The decision regarding what needs to be done involves various choices that require the employee to recognize the existence of and differences among a few easily recognizable situations.

Actions to be taken or responses to be made differ in such things as the source of information, the kind of transactions or entries, or other differences of a factual nature.

The work involves the execution of specific rules, regulations, or procedures and typically comprises a complete segment of an assignment or project of broader scope.

The work product or service affects the accuracy, reliability, or acceptability of further processes, services, and / or patient care.

Contacts

The personal contacts are with employees in the organization but outside the immediate department. People contacted generally are engaged in different functions, missions, and kinds of work, and may be representatives from various levels and departments within the organization, partner organizations, Tribal affiliate organizations, and employees of other healthcare organizations.

Personal contact, at this level, may also made with members of the general public, as individuals or groups, in a moderately structured setting.

The purpose is to obtain, clarify, or give facts or information regardless of the nature of those facts; i.e., the facts or information may range from easily understood to highly technical.

Work Environment / Physical Demands

The characteristic demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.

The employee must regularly lift and / or move up to 25 pounds and frequently lift and / or move up to 50 pounds. The employee is occasionally exposed to outside weather conditions.

The noise level in the work environment is usually moderate.

Local and moderate out-of-town travel is required.

Supervisory and Management Responsibility

This position has no formally assigned supervisory responsibility or authority. The employee is responsible only for the performance of their own assigned work.

They may be asked to train new employees in the fundamentals of the jobs or to participate in cross training of other employees in the department, but such assignments do not include the on going authority to assign and review work of other employees or to recommend or take corrective action with regard to other employees' performance.

Education / Experience / Certificates / Credentials

Bachelor’s Degree and one (1) year of experience, or associate’s degree and three (3) years’ of experience, or high school diploma or GED and five (5) years of progressively responsible experience.

Progressively responsible work experience may be substituted on a year-for-year basis for college.

Must successfully pass a criminal and background check, and a pre-employment drug screen.
