Kitsap Mental Health Services is seeking a dedicated Care Coordinator to join our multidisciplinary team. The Care Coordinator will demonstrate clinical skills and experience to plan, develop, coordinate, and provide treatment and support services to program clients.
This role also includes providing education, consultation to families, and crisis intervention services to students attending our embedded school programs.
Primary Responsibilities :
- Provide care coordination services for an assigned group of clients, including crisis intervention and case management, while coordinating directly with schools and school district personnel.
- Participate in interventions to address school expectations, including chronic absenteeism, and assist in creating plans to address academic, social, and emotional issues.
- Assist clients in identifying and building natural supports. Plan and schedule regular client-centered team meetings with clients and supports present.
- Educate and support clients' families, advocate for clients' rights and preferences, and consult with families and community agencies to maintain coordination in the treatment process.
- Develop, write, implement, evaluate, and revise overall treatment goals and assessments in collaboration with the treatment team, including the CANS assessment.
- Assist and support clients in finding and effectively using a primary care physician and a dentist, and develop health self-management skills.
Coordinate psychiatric care with medical care.
- Document client progress to maintain a permanent record of client activity according to established policies and procedures.
- Assist clients with gaining resources and education to support their autonomy and basic needs.
- Provide information to clients on substance use issues and coordinate treatment with internal and external substance use treatment providers.
Minimum Qualifications :
EDUCATION : Bachelor’s Degree in Psychology, social services, or behavioral health field.
EXPERIENCE : Entry level (no prior related work experience)
This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.
LICENSURE : Agency Affiliated Counselor Registration.
Ability to become CANS certified within first 30 days of employment.
Preferred Qualifications :
EXPERIENCE : Experienced (minimum 2 years of job-related experience)
The capacity to work well with children and families. Educational or classroom management experience preferred.
