We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).
Job Summary*
As a Hospice RN Case Manager, you'll play a crucial role in planning, organizing, and directing hospice care. This position requires a blend of nursing expertise, community health education, and a deep commitment to end-of-life care.
You'll be the cornerstone in building a bridge between our organization's resources and the needs of patients and families in their homes and communities.

- Core Responsibilities*
- Conduct patient and family assessments
- Develop and implement personalized care plans
- Provide skilled nursing care and pain management
- Coordinate with interdisciplinary team members
- Maintain accurate patient records
- Participate in on-call duties
- Qualifications*
- We're excited to train RN's that are passionate about hospice care*
- RN license
- 1-2 years recent acute care experience (preferred)
- 1-2 years home care experience (preferred)
- Valid driver's license and reliable vehicle
- Strong communication and interpersonal skills
- Empathy and comfort with end-of-life care
- Work Environment*
- Mix of facility and home-based care
- Average caseload : 12-15 patients
- Monday Friday, 8am-5pm (some on-call and weekends)
