The Executive Director will be accountable for providing strategic leadership and direction for Post-Acute Care Services and Physician Quality Initiatives.
This position reports directly to the Vice President / Chief Medical Officer.# This position includes a comprehensive understanding of post-acute care, as well as the operational, financial, programmatic and medical / clinical quality components of the services.
The Executive Director provides direct direction and leadership in the development, implementation, oversight, and evaluation of Physician Quality Measures.
In defining and monitoring the physician key measures of successful service delivery, the Executive Director will work closely with the CMO in identifying, developing, defining and implementing physician-driven quality measures and identifying the necessary structures, relationships, and resources needed to achieve the strategic goals.
Assist in the development and management of the physician quality dashboard.### Demonstrates a strong understanding of quality measures and have an understanding of the FH quality system (Crimson) continumn of care.
The Executive Director is accountable for leading, planning, implementing, evaluating, and integrating post-acute care services over the continuum of care for Forrest Health System working closely with a variety of stakeholders, including physicians, skilled nursing units, rehabilitation unit, outpatient therapy, home health, hospice, palliative care and other post-acute care facilities as applicable.
Demonstrates knowledge and skills to appropriately communicate and interact with the patients, physicians, staff, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs.
Performs other duties as assigned by CMO. # Performance expectations : # Work with and communicate with Vice President / CMO regarding program development, and future needs of post-acute care and physician initiatives Work with Vice President to develop and implement physician dash board / score card, reflecting quality and performance metrics using the Crimson Continuum of Care platform or updated Optum EIS platform.
Work to promote and educate all stakeholders regarding the overall initiatives, goals, priorities, and measurable outcomes of services Work with physicians on identifying, developing and implementing physician-driven quality measures.
Run reports and gather data as assigned related to physician quality. Promote strong physician-hospital relationships through favorable alignment models across our health system.
Promote strong understanding of quality measures and quality systems (Crimson) within the FH system. Be responsive to emerging reimbursement trends for services related to the areas of responsibility.
Understand the growing trends and value of leveraging technology to improve data collection. Maximize an organization#s value through physician-driven services and governance structures Promote strong service initiatives to provide high quality care for all customers Directs and provides guidance to other Directors to effectively allocate resources and services based on referrals, patient volume, program goals and objectives.
Assesses all services in post-acute care to assure collaboration and effective / efficient operations. Ensures development of post-acute service line to improve the appropriate flow of patients across the continuum of care.
Qualifications : Education / Skills###### ########## Bachelor Degree in Nursing, Business Administration, Health Care Administration, or a related field ########## required.
Master#s degree in Nursing Administration, Hospital Administration, Health, Business, or ####### Public Health preferred.
Work Experience : ## A minimum of Five years of healthcare experience or knowledge in post-acute care services.## A minimum of Five years experience with responsibility for staff supervision and management required.
