Market Director of Case Management
We are searching for a motivated Market Director of Case Management at an acute care facility in outside of Nashville, TN!
Job Summary
The Market Director of Case Management will provide Case Management leadership and management for hospitals within a market.
The position will be responsible for management or delegation of day-to-day operations including directing and planning.
Formulate and implement operational strategies and initiatives in the case management program. Ensure staff competencies in all areas of care management to include utilization review, transition / discharge planning, social work services.
Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes, and issues.
Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.
Minimum Qualifications :
Must-Haves
1. Bachelor’s degree in Nursing - Required
2. State of Tennessee or Multi-State Registered Nurse licensure - Required
3. Minimum four years management experience directing a Case Management Department in an acute care hospital.
4. Working knowledge of Care Management department functions including Joint Commission and Conditions of Participation (COP) requirements.
5. Good understanding of Medicare, Medicaid, and other third-party payment systems.
Nice-To-Haves
1. BLS certification from the American Heart Association (AHA) or American Red Cross (ARC) - Required within 30 days of hire, if not current upon hire.
2. Master's degree in Nursing, Healthcare Administration or Business Administration
3. Certification in Case Management such as CCM
Salary Base : $112K- $130K plus bonus incentive
