As the Market Director of Case Management, you will lead and manage case management services across Highpoint Health hospitals.
Your responsibilities will include day-to-day operations management, strategic planning, and implementation of case management initiatives.
You will ensure staff competency in areas such as utilization review, discharge planning, and social work services, while aligning resources with system-wide goals.
Additionally, you will collaborate with physicians and other stakeholders to deliver evidence-based care and reduce clinical variations.
Partner with Hospital Executive Leadership, Nursing, Medical Staff, and other department stakeholders to ensure safe and effective patient care.
Qualifications :
The position will be responsible for management or delegation of day-to-day operations including directing and planning. Formulate and implement operational strategies and initiatives in the case management program.
Ensure staff competencies in all areas of care management to include utilization review, transition / discharge planning, social work services.
Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes, and issues.
Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.
Requirements :
- Bachelor’s degree in Nursing
- State of Tennessee or Multi-State Registered Nurse licensure
- Minimum four years management experience directing a Case Management Department in an acute care hospital.
- Working knowledge of Care Management department functions including Joint Commission and Conditions of Participation (COP) requirements.
- Good understanding of Medicare, Medicaid, and other third-party payment systems.
