Activities Assistant / CNA
Join the HopeWest PACE team as the Activities Assistant / CNA and be responsible for completing recreational assessments, creating, implementing and updating care plans for participants, and providing the highest quality of skilled and non-skilled care in the day center.
The Activities Assistant / CNA will follow the established plan-of-care for each participant that may include ADL activities, meal preparation, monitoring vital signs, and communicating any concerns or changes to the proper IDT member.
What is PACE?
PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes.
There are 163 PACE programs, serving more than 72,000 participants in 32 states.
The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.
What You Bring :
- Current Colorado CNA license maintained throughout employment.
- 6 months experience as a CNA preferred.
- Experience working with older adults preferred.
- Experience in activities planning preferred
- A genuine spirit of caring and compassion
- Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation if working in the community setting
- Ability to work collaboratively and communicate effectively

- Competitive pay structure starting at $21.00 per hour.
- Monday-Friday, 8 : 00am-5 : 00pm schedule
