Services provided include Psych Express Care (2-3 hour intervention), Psych Crisis Care, and Psych Progressions (Outpatient Therapist Services).

MIND 24-7 not only enhances the quality of patient care, but reduces pressure on the mental health system, reducing costly emergency room visits, in-patient treatment, and readmissions for crisis care.

In the first year, with now over 450 team members, the company has opened three facilities in the Greater Phoenix are and has served over 10,000 individuals in need of care.

Immediate rapid expansion into additional states is planned for the coming year.

The Patient Navigator provides case management and care coordination as part of a multidisciplinary clinical team; Navigator, Nurse, Behavioral Health Medical Professional (BHMP), Patient Advocate and Patient Care Coordinators.

The Patient Navigator will be responsible for coordination of care activities, follow up activities, resource development, assessment, and treatment planning, staffing cases with clinical team members and discharge planning.

ESSENTIAL JOB FUNCTIONS

  • Monitors daily activities and coordinates care with crisis center and outpatient staff
  • Will communicate with the clinical team to ensure client's goals and objectives are clear
  • Coordinates with family members and team on service delivery methods and options
  • Coordinates staffing with other team members
  • Coordinates with clinical team regarding client appointments
  • Provides recommendations to identify how to assist patient with any social determinant needs identified
  • Works with member and family to educate on needed services as well as advocate for additional services identified
  • Performs case management duties as defined in each member's individualized treatment plan
  • Completes all needed documentation for behavioral services provided
  • Other duties as assigned
  • Will attend supervision sessions as required.
  • Will complete all other required trainings as assigned.
  • Depending on setting, may also :
  • Float among various clinical services, where qualified and competent

SUPERVISORY RESPONSIBILITIES

  • Reporting Structure : This position reports to the Lead Patient Navigator.
  • Supervisory Functions : No.

POSITION QUALIFICATIONS

Minimum Qualifications :

  • High School Diploma or equivalent and 4 years of behavioral health experience
  • Associate degree in Psychology or related field and 1 year of behavioral health experience
  • Bachelor's Degree in Psychology or related field
  • Level 1 Fingerprint Clearance Card, or ability to obtain upon hire

Preferred Qualifications :

  • Master's Degree in social work or counseling
  • Associate license of LAC, LMSW, or equivalent with the state of Arizona

WORK ENVIRONMENT

  • Typical medical office environment
  • This role routinely uses standard office equipment such as computers, phones, and photocopiers
  • Travel to company and each clinic location as may be necessary
