Job Description
LGHP Geriatrics is seeking a full-time (0.8 FTE) Nurse Practitioner to join their nationally recognized team. The Geriatrics team, consisting of 38 physicians and nurse practitioners, provides high quality care to the frail geriatric population in a variety of service areas including the outpatient office, nursing home, hospital consult service, and home care.
Our team focuses on providing the geriatric population of Lancaster County, PA with the highest standard of care tailored to meet individual patient needs.
Full time position working primarily in a fast-paced, nursing home environment, with consideration for coverage in an office setting or in our Home Care program.
Caring for a wide range of patients from post-acute rehab to long-term care and hospice residents. Responsibilities include acute and chronic illness management and routine rounding with a flexible schedule.
Hours : Full time position ( 32 hours / week ). Monday - Friday, 8am-5pm. Shared call rotation with other colleagues in the practice - weeknights and holidays.
Job Requirements
Nurse Practitioner
- Valid Certified Registered Nurse Practitioner license and Certificate of Prescriptive Authority in Pennsylvania
- Valid Pennsylvania licensure as a Registered Nurse
- Certification by ANCC or other qualifying agency in area of specialty required (FNP or AGPCNP required for this NP position)
- Minimum one-year experience as a Nurse Practitioner is preferred
