• The required availability for this position is Monday–Friday, 8 : 00am–4 : 30pm (no weekends or on-call).\n
• Two openings available – ASAP start!\n\nRegistered Nurse (Hospice) Responsibilities\n
• Complete admission, recertification, routine, PRN, and death visits (5–7 per day).\n
• Conduct comprehensive assessments for high-risk members.\n
• Collaborate with physicians, social workers, and care teams to implement individualized care plans.\n
• Coordinate care transitions and community resources.\n
• Maintain detailed records of clinical, functional, and fiscal outcomes.\n
