JOB SUMMARY :

The Music Therapist is responsible for assessing and developing a Care Plan for patients age, emotional wellbeing, physical health, communication ability, and cognitive skills through musical responses to meet the needs of patients and their families, promoting the quality of life for terminally ill patients and their families and participation of the development of the hospice interdisciplinary team care plan.

The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families / caregivers from Luminary Hospice of Indiana, LLC for up to 13 months following the death of the individual hospice patient.

JOB RESPONSIBILITIES & DUTIES :

  • Assesses hospice and palliative care patients of all ages to identify needs and provide interventions that meet individual needs.
  • Identifies and documents problems, and sets developmentally appropriate goals, time frames, and interventions.
  • Provides and documents direct patient care according to the Care Plan, within set target dates, and according to all departmental policies.
  • Designs music sessions for individuals and groups based on client needs. Provides basic and specialized teaching as required in the Plan of Care and documents the response of patients and / or family in the patient's record
  • Regularly evaluates patient's progress, takes prompt action when any change in the patient's condition is noted or reported, and terminates care when goals of management are attained for patients of all ages.

Participates in treatment planning, ongoing evaluation and follow-up.

Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.

Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family / caregivers.

Plans for Bereavement Program development, expansion, and refinement through annual evaluation of services

Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources.

Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.

Perform additional job duties as assigned

JOB REQUIREMENTS & QUALIFICATIONS :

Bachelor's degree in therapy or related fields, required. Master's degree in Music Therapy or related healthcare field, preferred.

Maintain an active Board Certification in Music Therapy for the state practicing. Maintains an active CPR certification.

At least 2-5 years of experience in the healthcare management, hospice or palliative care and / or training in end-of-life care, preferred.

At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred. Knowledge hospice care and the services provided to patient and family / caregiver through an interdisciplinary group.

Proficient in MS Office applications and ability to learn department and job-specific software systems

Demonstrate organizational skills. Demonstrate effective verbal and written communication skills

Demonstrate analytical skills when problem-solving. Demonstrate high attention to detail and a high degree of accuracy.
