Direct Support Professionals provide opportunities for people to grow as individuals and members of the community. This position is responsible for supporting individuals in daily tasks of living, and providing social, recreational and community-based activities.
Services are delivered in compliance with HHSC service definitions and principles for both Home and Community Based Program (HCS) and Texas Home Living Program (TxHmL) as well as meeting community standards.
We are seeking Team Members that are self-driven, eager to learn and grow with the program and willing to support all aspects of the program, both in the homes and in the day services program.
You will get to :
- Able to be a role model to Individuals. Provide hand over hand assistance and / or hands on assistance with daily living skills
- Assist individuals with their finances through money management training, money identification and making purchases / counting change
- Able to provide supports to Individuals who exhibit behaviors, both physical and verbal; able to follow and implement Behavior Support Plans and document accordingly
- Assist individuals with meal preparation, to include shopping, following recipes and assist in the cooking process
- Provide transportation to and from community outings, community employment and other scheduled events while meeting all transportation guidelines
- Monitor, assist with and document individuals’ self-administration of medication
- Ensure individuals safety by participating in fire drills and evacuation drills
- Maintain cleanliness of the home by following daily, weekly and / or monthly checklist provided
- Maintain an environment free of hazards; completing work orders when necessary and reporting immediate concerns to the Management Team
- Provide accurate documentation of services provided, within 48 hours, through paper documentation as well as the use of an Electronic Health Record
- Utilize EVV devices for services provided, when necessary
- Able to learn and utilize Adaptive Aid Equipment, including wheelchairs and Hoyer Lifts
- Work cooperatively with others and communicates with everyone on the team
- Work independently and carry out assignments to completion with minimal supervision
- Provide exemplary customer service to the Individuals and Family Members
- Maintain punctuality and consistent work attendance, providing at least two weeks’ notice for any time requested
- Able to show competency in ALL required procedures of Prevention and Management of Aggressive Behavior (PMAB) program annually as required by the Texas Administrative Code.
- Provides professional representation of Hill Country MHDD Centers at meetings, seminars, consortiums, public events, stakeholder and other interaction opportunities as assigned.
- Travels as necessary to ensure delivery and completion of assigned duties. May drive agency or personal vehicle as necessary in performance of job duties.
- Guards client confidentiality by being knowledgeable of and adhering to HIPPA regulations.
- Maintain current training requirements in Relias, as well as, other trainings as assigned either in person or virtual.
