Key ResponsibilitiesProvide transition of care calls and conduct assessments to identify member needsCollaborate with interdisciplinary care teams to develop and monitor individualized care plansManage a caseload effectively while ensuring adherence to quality metrics and documentation requirementsRequired QualificationsCurrent, unrestricted RN license with Compact State Licensure3+ years of diverse clinical experience, preferably with acutely ill membersKnowledge of utilization management, quality improvement, and discharge planningExperience with Microsoft Office applicationsCurrently residing in the Mid-West and able to work with CST and EST Time Zones
