Kuakini Medical Center Description of Position
Performs various tasks and activities to providepatients and clients with access to resources to meet their needs for care.
Supports the execution of discharge plans and continuity of care and plan ofcare outcomes for patients and families by coordinating with resources, arranginglogistical elements, communicating with third-party providers and fulfillingprocedural requirements.
Assists MedicalSocial Workers and Care Management Coordinators with finalizing interventions,referrals and services. Providesinformation and education and communicates with patients and families, asassigned.
Qualifications and Requirements
Requires high school diploma or equivalent. A Bachelor degree in SocialWork or a related major subject is preferred. Preferred experience is previouswork performing functions similar to a Social Work Assistant or three (3) yearswork experience in a direct patient care setting or with a social servicesprovider;
or any combination of education and experience which would providethe necessary knowledge, skills and abilities to meet the minimum qualificationsto perform the essential functions of this position.
Starting Hourly : $23.78
