The Family Advocacy & Support Case Manager (School Based) assesses the social service needs of participants whose needs range from routine to complex.

The Case Manager provides a range of direct individual and group engagement, assessment, service coordination, referral, psychoeducation, skill-building, and supportive services within the agency’s policies, procedures, and practices.

This position reports to the assigned Supervisor.

Core Responsibilities :

  • Provides a range of case management services to assigned participants, which may include maintaining a caseload or providing services in collaboration with another assigned provider.
  • Arranges and / or provides services needed to meet the immediate needs of participant(s).
  • Works in partnership with participant, and family members if appropriate, to assess strengths, functioning and natural support systems.
  • May develop service / treatment plans and goals with assigned participant(s) / family(s) or support the implementation of service plan goals developed by another assigned provider.
  • In collaboration with assigned participant(s) and any other involved service providers, analyzes and evaluates progress in the service / treatment plan and modifies when necessary to meet the needs and goals of the participant(s).
  • Assumes responsibility for comprehensive case management which may include : needs assessment, service planning, case coordination, service brokerage, linkages with community resources, psychoeducation, skill-building, and monitoring progress toward participant(s) goals and objectives;

makes recommendations for on-going services when necessary.

  • Coordinates services and attends and actively participates in meetings with other agencies, systems, and community providers to meet the needs of the participant(s).
  • Schedules appointments to the participant(s) home or a setting most agreeable to the participant(s) at a time that is convenient for involved parties.
  • Provides and / or arranges transportation for participants.
  • Develops aftercare service / treatment plans with the participant and ensures that any recommended services or providers are in place prior to participant(s) being discharged.
  • Reports any suspected abuse or neglect to the Department of Children and Family Services per the Abused and Neglected Child Reporting Act.
  • Attends regular supervision staffing’s with designated program supervisor and attends clinical supervision with clinical team when deemed necessary.
  • Meets the documenting, record-keeping, and billing requirements of the program.
  • Works cooperatively with other personnel on all levels.

Candidate qualifications :

Bachelor’s degree in social work or another human service field required.

Programs that do not have contract, regulatory, accreditation or other standards regarding case manager qualifications may consider candidates who have a minimum of 3 years of closely related experience but who do not have a college degree.)

Demonstrates effective verbal communication skills to convey information and ideas in the English language to various audiences, in both one-to-one and group settings.

Additionally, proficient speaking in a second language may be required or preferred based on program assignment.

  • Demonstrates proficient written communication skills to document and convey information accurately and within established timeframes.
  • Demonstrates effective interpersonal skills to establish and maintain rapport and professional relationships with co-workers, and team members, and participants / families enrolled in services.
  • Demonstrates initiative, consistent follow-through, and the ability to manage multiple tasks and meet deadlines.
  • Demonstrates intermediate computer skills, including Microsoft Word and Outlook; demonstrates proficiency in participant electronic data systems used by the program within 60 days of hire, depending on program assignment.
  • Demonstrates respect and acceptance of diverse individuals and communities served by, working in partnership with, or employed by Brightpoint including but not limited to race, gender, ethnicity, social-economic status, and LGBTQIA+ populations.
  • Must have a valid driver’s license, current auto insurance and access to a reliable vehicle to execute the position’s responsibilities.

Job details :

  • Compensation : Range starts at $19 / hr; offers are commensurate with experience.
  • Benefits : Medical / dental / vision insurance, 3+ weeks paid time off in the first year including the week between Christmas and New Year’s Day (PTO increases with tenure);

  • Location : Most work will be completed at the Rockford Family Peace Center with some need to attend community-based meetings or events.
  • Schedule : Full-time, hourly; general business hours with some flexibility to accommodate clients’ scheduling needs.
  • Student Loan Forgiveness : Brightpoint is an approved agency for Public Service Loan Forgiveness (PSLF).
