Responsibilities for Physical Therapist
Based on findings from initial evaluation, develop an individualized plan of care to facilitate optimal performance using standardized tests and measures as evidence for improvement.
Identify need and make referrals to other disciplines to address the comprehensive needs of the patient
Monitor patients' progress and adjust treatments accordingly.
Provide supervision for Physical Therapy Assistants, students and support personnel in accordance with state license requirements and professional standards
Perform other duties as assigned and require to provide quality care to residents
Qualifications for Physical Therapist
- Experience providing physical therapy to patients in a geriatric long term care facility
- Graduated from an accredited physical therapy program
- Current Physical Therapy license for state of Delaware
- Current CPR certificate
- This position requires good organizational, time management and communication skills.
