The Care Coordinator provides intensive clinical care coordination to the youth and family as well as counseling and behavioral / emotional support.

Your primary role as the Care Coordinator is to coordinate care among all entities involved with the child, ensuring everyone is aligned with the goals, and help build natural supports and enhance the treatment team.

Care Coordinators engage clients in meaningful and authentic relationships while providing a safe, predictable, and positive environment that aligns with our clients’ values and cultures.

Services will be provided almost exclusively in the community in families’ homes, group homes, foster homes, schools, etc.

ABOUT THE KATIE A SERVICES

Seneca serves Santa Clara County children / youth by offering a focus on managing mental and behavioral health needs and increasing placement stabilization.

The Katie A. Services program provides culturally-competent, team-driven, and strength-based unconditional care for children and families who struggle with profound needs and difficulties.

Katie A. Services services dually focus on the child and family team meeting process that focuses on building a strong team to collaboratively address the needs of the child and family.

Services are provided at the program site and in the community, including schools, and family homes.

Responsibilities :

Promptly meet with clients and caregivers on a regular basis to further treatment goals utilizing culturally informed and best practice modalities

Cultivate and maintain relationships with community members and providers

Complete assessment and evaluation process of clients in order to develop and implement the care plan

Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate

Chart mental health notes, care plans, outcome measures, incident reports, and other documentation accurately and in a timely manner

Maintain availability for crisis intervention services, such as on-call rotation, if applicable

Participate in individual and / or group supervision

Maintain strict confidentiality of client and business operations information

Qualifications : REQUIRED

REQUIRED

Master’s degree in social work / psychology / counseling

Registered with California BBS : ASW, AMFT, APCC, or full licensed

Flexible schedule with the ability to work some evenings and weekend hours as needed

Must be at least 21 years of age

Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy

TB test clearance, fingerprinting clearance, and any other state / federal licensing or certification requirements

Proof of up-to-date COVID-19 vaccination

PREFERRED

Bilingual in Spanish / English

Experience working with children / youth and families with complex and enduring needs

Experience implementing family-focused services

SKILL OF AN IDEAL CANDIDATE

Ability to work effectively with a diverse population and workforce

Strong organizational, time management, and writing skills

Ability to work as part of a multidisciplinary team

Schedule :

Full-timeMonday - Friday; 10am - 6 : 30pm

Occasional 11am - 7pm schedule, scheduled ahead of time

Benefits :

Starting at $84,872 - $98,872 per year, commensurate with experienceAdditional $4000 salary increase for BBS licensure
