Nurse Practitioner
This rewarding community based position is a full-time (Monday-Friday) Nurse Practitioner or Physician Assistant role where you’ll provide direct care to adult-geriatric patients with an average age of 85.
Typically, you’ll see 4-6 patients a day (4.5 scheduled days per week) for 45mins-1hr each and you’ll be doing follow-ups from hospital appointments, checking meds, ensuring they’re adhering to their treatments, reviewing medical records, bandage changes, wound care, and you may occasionally draw blood for labs or insert IV fluids.
The point of your visit is to provide treatment for whatever the patient needs and fill in the gaps until the member can see their PCP.
In total, you’ll have a panel of up to 200 patients within a particular geography, so you will get to know them. Your patient panel is based on proximity to your home and would be comprised of members who are within approximately one hour or less from your address.
These patients are also primarily Medicare Advantage patients.
