Job Summary

The Case Manager is responsible for coordinating comprehensive care and support services for clients within the behavioral health program.

This role involves assessing clients' needs, developing individualized service plans, linking clients to appropriate resources, and monitoring their progress throughout their treatment and recovery journey.

Our ideal candidate also works to foster an inclusive, respectful community that promotes personal growth and recovery, with a special focus on the unique needs of LGBTQIA individuals.

Availability

Shifts may include Monday through Friday, 9am-5pm, not remote

Key Responsibilities

  • Conduct comprehensive assessments to identify the needs and strengths of clients.
  • Develop and implement individualized service plans in collaboration with clients and the treatment team.
  • Coordinate with various community resources, healthcare providers, and social services to ensure clients receive the necessary support.
  • Monitor client progress, adjusting service plans as needed, and providing ongoing support to help clients achieve their recovery goals.
  • Advocate for clients, ensuring they have access to services and resources that support their mental health and well-being.
  • Maintain accurate and confidential client records, documenting all interactions, services provided, and progress toward goals.
  • Facilitate communication between clients, families, and the treatment team to ensure a cohesive approach to care.
  • Provide crisis intervention and support as needed, connecting clients to emergency services or higher levels of care if required.
  • Educate clients and their families about available services, treatment options, and strategies for maintaining recovery and wellness.
  • Ensure compliance with all state and federal regulations related to case management and behavioral health services.
  • Meet with clients Monday through Thursday for 45 minutes each week.
  • Ability to attend weekly SUD / Case management meetings and trainings as scheduled and as needed.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, or a related field; a master’s degree is preferred.
  • Minimum of 2 years of experience in case management, preferably in a behavioral health or social services setting.
  • Strong knowledge of community resources and services available for individuals with mental health and substance use disorders.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Commitment to client-centered care and advocacy.
