Weekend Hospice (RN) Case Manager - Part-Time
A Part-Time Weekend Hospice (RN) Case Manager is needed in Atlanta, GA.
Titan Placement Group is currently seeking a compassionate and experienced Weekend Hospice RN Case Manager to join a reputable healthcare team in Atlanta, GA.

Salary and Benefits :
- Competitive hourly pay : $41 to $43 per hour
- Schedule : Every other weekend, Saturday and Sunday (10-hour shifts)
- Mileage Reimbursement : $0.50 per mile
- Cellphone Allowance : $60 per month
Responsibilities :
- Work every other weekend : Saturday and Sunday (10-hour shifts)
- Provide hospice care services to patients in their homes
- Manage a case load of hospice patients, ensuring quality end-of-life care
- Collaborate with an interdisciplinary healthcare team
- Conduct thorough patient assessments and develop care plans accordingly
- Maintain detailed patient records using the provided EMR system
Requirements :
- Clear and active RN license in the state of Georgia
- Preferred experience in Hospice, Home Health, ICU, CCU, Oncology, End-of-Life Care, or MedSurg
- Compassionate and dedicated approach to patient care
- Strong communication and organizational skills
