Responsibilities
Clinical Patient Navigator for the Breast Surgery Program participates on the multi disciplinary team as an advocate, educator and counselor for Breast Surgery patients.
Coordinates care through the continuum of diagnosis of breast cancer and benign breast diseases in collaboration with surgical team.
Responsible for ensuring all patients receive timely and comprehensive services, including follow up with radiology, medical oncology, radiation oncology, social work, physical therapy and additional services, as needed.
Serves to identify barriers and find resources to assist patients. Serves as a liaison throughout the network and in the community regarding available services.
- Assist patients in understanding their diagnosis, treatment options and available resources.
- Manage recredentialing for NAPBC
- Assist w Biopsy scheduling
- Educating patients regarding drain teaching and post-operative instruction
- Assist Departmental surgeons with research projects
- Helps track patients with abnormal findings in need of additional tests / biopsies
- Serves as liaison between patients and care providers
- Facilitate appointment and support services
- Coordinate collection of data and tracking of outcomes with cancer registry staff.
- Coordinates care conferences preparing concise patient summaries for use by care team. Documents recommendations using standard protocols
- Assist with outreach efforts and screening activities.
- Provides office coverage for Medical Assistant shortages
- Participate in Steering Committee and Multi-disciplinary Tumor Boards
- Assist with departmental Research projects
Qualifications
Education / Training / Experience : (indicate if required or preferred)
PA RN required, BSN preferred
At least 2 years of clinical experience and Patient education / care management preferred
PC proficiency required, Windows , Microsoft Office
Data base management and spreadsheet knowledge required
Familiarity with patient medical records and electronic medical record systems required
Experience with Breast Surgery Patient Management and / or oncology patient populations preferred
Excellent presentation and communication skills
Able to work in a self directed environment, ability to work with and lead teams required
Conditions of Employment
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check.
The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
