Responsibilities

Coordinate home care and / or hospice intake activities and resources.

• Complete referral process.
• Explain home health and / or hospice services and facilitate continuity of care.
• Assist with communication or payer sources. Determine benefit eligibility.
• Obtain, record and disseminate referral information.
• Document clinical records to ensure compliance with regulatory standards, agency policies and procedures.

Requirements

Licensure / Certification / Registration :

Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education :

Diploma from an accredited school / college of nursing OR required professional licensure at time of hire.
