Reentry Specialist
The Prison Reentry Coordinator is responsible for assisting formerly incarcerated individuals in their transition back into society.
This role involves developing reentry plans, providing resources, and supporting clients in overcoming barriers to successful reintegration.
Key Responsibilities
- Assessment and Planning
- Conduct comprehensive assessments of individuals' needs, risks, and strengths prior to release.
- Develop personalized reentry plans in collaboration with the individual and relevant stakeholders.
- Set realistic goals and objectives to address identified needs and reduce recidivism risk.
- Case Management
- Provide one-on-one case management and support to individuals during their reentry process.
- Maintain accurate and timely documentation of all interactions, assessments, and services provided.
- Monitor progress toward goals and adjust reentry plans as needed.
- Resource Coordination
- nbsp; Connect individuals with community resources, including housing, employment, healthcare, education, and counseling.
- Maintain up-to-date knowledge of available resources and services in the community.
- Coordinate with other service providers to ensure a holistic approach to reentry support.
- Support and Advocacy
- Provide emotional support and guidance to individuals navigating reentry challenges.
- Advocate for individuals to ensure they receive appropriate services and support.
- Assist individuals in overcoming barriers such as lack of identification, transportation, or access to basic needs.
- Education and Skill Building
- Facilitate workshops and training on topics such as job readiness, financial literacy, and life skills.
- Provide information and resources on rights and responsibilities after release.
- Empower individuals with tools and strategies to make positive choices and avoid recidivism.

- Build and maintain strong relationships with community organizations, employers, service providers, and stakeholders.
- Participate in community outreach activities to promote awareness of reentry services and support.
- Attend meetings, trainings, and professional development opportunities to stay informed about best practices in reentry.
- Follow-Up and Evaluation
- Conduct follow-up visits and check-ins to ensure ongoing support and stability.
- Evaluate the effectiveness of reentry plans and adjust strategies as necessary.
- Collect and analyze data to measure outcomes and improve reentry services.
Qualifications
- Education
- Bachelor’s degree in social work, criminal justice, psychology, human services, or a related field (required).
- Master’s degree in social work or a related field (preferred).
- Experience
- Minimum of 2-3 years of experience in reentry services, social work, corrections, or a related field.
- Experience working with justice-involved individuals and knowledge of the criminal justice system.
- Skills
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Excellent organizational and time-management skills.

Work Environment

Salary
$35,000 per year.
