Job Description :
Gentiva Hospice is seeking a compassionate and experienced Hospice RN Case Manager to join our team of dedicated clinicians.
In this role, you will provide direct patient care services, act as a liaison between physicians and patients / families, and collaborate with an interdisciplinary group to ensure comfort, peace, and dignity for our patients.
The ideal candidate will have a current RN license, 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology, and be comfortable providing care wherever the patient calls home.
