FTE : 1.0, Shift : Day, Schedule : Mon-Fri

This is a hybrid role. Must be located in Spokane, WA! Deaconess will be the home base hospital - will also serve in other post-acute settings.

Responsibilities

  • You will perform cross continuum care activities, which may include clinic, provider practice, urgent care, phone care agency, skilled care, or other sites of care, as appropriate
  • You will provide customer service to the patient, family, physician / APP, and care team members to include discharge milestones and progression to next level of care
  • You will model and orchestrate consistent, professional communications and maintain consistent and proactive communication patterns with colleagues and staff
  • You will delegate responsibilities appropriately but remains accountable for outcomes
  • You will ensure that the patient and family experiences medically appropriate transitions of care

Requirements

  • Master’s degree in Social Work
  • Licensed in the state of Washington
  • Licensed Independent Clinical Social Worker (LICSW); OR Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain their LICSW within four (4) years
  • Minimum three (3) years Hospital or equivalent experience
  • Health industry knowledge preferred
  • Healthcare financial assistance knowledge preferred
  • Leadership experience a plus

Position Summary

The Hospital Case Manager RN is responsible for high quality cross continuum care coordination. This individual works with physicians / APP, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care and services, to improve population health, and to achieve collaborative practices that exemplify MultiCare Health System’s commitment to patient centered care and community engagement.

Responsibilities

  • You will ensure that the patient and family experiences medically appropriate transitions of care, and an integral and essential component of the care delivery process
  • You will advocate for patients and families, including transition to lower level of care when inpatient care is no longer needed
  • You will partner with and supports physicians / APP in patient care activities and care transitions
  • You will utilize critical thinking in all interactions
  • You will be accountable for transitions of care and care across the continuum

Requirements

  • Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service may be considered in lieu of education agreement
  • Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
  • Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
  • Four (4) years recent hospital case management experience
  • Five (5) years of recent clinical hospital / ambulatory experience
  • Healthcare financial and reimbursement knowledge preferred
  • Leadership and emergency services experience a plus
  • Consideration may be given to internal candidates not meeting the minimum qualifications

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off.

As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position.

The pay scale is $96,689.00 - $161,160.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to : skill set, level of experience, and certification(s) and / or education.

If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
