Patient Care Coordinator - Nephrology

We are hiring at Starling Physicians, a VillageMD company!

Starling Physicians is seeking a full-time Patient Care Coordinator to join our Nephrology / Internal Medicine department.

Candidate must be willing to travel between our Hartford and Wethersfield offices.

  • Responsibilities will include, checking and maintaining physician schedules and organizing patient flow; Charting; relaying instructions to patients / families;
  • answering calls and providing pertinent patient information. Fulfills clerical responsibilities as assigned; sending / receiving medical records;
  • obtaining lab / x-ray reports, hospital notes; providing and processing referral information; completing forms / requisitions as needed;

scheduling appointments; verifying insurance coverage and patient demographics.

Key Accountabilities

Performs patient care responsibilities as assigned which may include : checking and maintaining physician schedules and organizing patient flow;

Charting; relaying instructions to patients / families; answering calls and providing pertinent patient information.

Fulfills clerical responsibilities as assigned which may include : sending / receiving medical records; obtaining lab / x-ray reports, hospital notes;

providing and processing referral information; completing forms / requisitions as needed; scheduling appointments; verifying insurance coverage and patient demographics - ensuring information is complete and accurate.

Refills as authorized by physician via fax, email, phone or electronic health record (EHR).

Review all abnormal lab results, flagging / coordinating urgent results for immediate physician review.

Contacting patients with new treatment / medication / lab instructions as instructed by physician.

Communicating with outside medical treatment centers regarding patient issues and relaying physician instructions to same.

Coordinate patient care by triaging patient phone calls relaying pertinent information / symptoms and requests to the physicians and performing appropriate follow-up action on all items as directed by physician.

Schedules urgent patient appointments, procedures, tests, etc. as necessary.

Documents all activities / interventions, patient / family responses, medication dispensed / prescribed., in the patients’ medical record.

Schedule procedures and tests (including acquiring lab results, dictation, physician orders and patient instruction related to the scheduled test / procedure).

Responsible for assuring that patient files are updated / current providing pertinent information to the physician in a timely manner.

Maintains strict patient confidentiality.

Adheres to OSHA regulations.

Performs other duties as requested.

Maintains patient portal.

Relationships (Reports to, Supervises)

Reports to the Team Leader, Clinical Supervisor, or Practice Manager.

Establishes and maintains effective working relationships with patients, co-workers, physicians, and the public.

Qualifications (Education, Experience, Additional Skills & Requirements)

High school graduate or equivalent.

Must possess medical assistant certification or equivalent experience.

Good computer and math skills.

Minimum 3-5 years experience as a Medical Assistant.

Must be detail oriented with good interpersonal and customer service skills.

Ability to work effectively and professionally under pressure.

Ability to work in a team and to share responsibilities and duties.

Ability to resolve interpersonal conflict in a straightforward and timely manner.

Ability to set priorities, be organized and be a self-starter.

Ability to be friendly, empathic and an adept communicator.

Ability to treat both staff and patients with respect

Working Conditions, Physical Requirements

Support function will occur within various clinical settings and work spaces.

Incumbent may be required to work at any facility based on operation need, and schedule may vary.

Travel to various practice locations will require ability to drive, valid driver’s license and use of own reliable transportation.

Physical activity will include walking, standing, and sitting.

Physical demands may also include lifting and carrying equipment up to 30 lbs. or providing physical assistance to patients .

Other physical requirements include speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes;

vision adequate to read correspondence, computer screen, forms, etc.; and good manual dexterity.
