Job Summary : We are seeking a dedicated and compassionate Primary Care Registered Nurse (RN) to join our healthcare team in New Hampshire.

The ideal candidate will provide comprehensive nursing care to patients in a primary care setting, collaborating with physicians and other healthcare professionals to promote optimal patient health and well-being.

Key Responsibilities :

  • Conduct patient assessments, including medical history and physical examinations.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor and document patient progress, response to treatments, and any changes in condition.
  • Educate patients and their families on health management, disease prevention, and wellness strategies.
  • Provide support and guidance to patients regarding lifestyle changes and self-care practices.
  • Assist with diagnostic tests and procedures as needed, ensuring patient comfort and safety.
  • Maintain accurate and timely documentation in electronic health records (EHR).
  • Participate in quality improvement initiatives and adhere to hospital policies and procedures.
  • Foster a positive and supportive environment for patients, families, and colleagues.
  • Stay current with nursing practices, advancements in healthcare, and relevant regulations.

Qualifications :

  • Current Registered Nurse (RN) license in New Hampshire.
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
  • Minimum of 3 years of experience in a primary care or outpatient setting preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Proficient in using electronic health record (EHR) systems and other healthcare technologies.
  • BLS and ACLS certification required.
