Schedule : Full Time, Day shift

Service Area : Tigard / Tualatin / Sherwood / Lake Oswego / West Linn areas

Job Description :

Works under the direction of a registered nurse to provide quality nursing care to patients in their homes. Assists with collection of assessment data and provides input to the planning of care.

Implements plan of care within the scope of licensure. Performs all duties in a manner which promotes team concept and reflects the Providence mission and philosophy.

Required Qualifications :

• Graduate of an accredited LPN / LVN school of nursing
• Within 30 days of hire : National Provider BLS - American Heart Association
• Upon hire : Oregon Licensed Practical Nurse
• Upon hire : Washington Licensed Practical Nurse - if practicing within this state
• Upon request : Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving.

Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information

• Upon hire : Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
• Upon hire : In compliance with the Providence St. Joseph Health - OR, criminal background check
• 1 year experience in nursing; long term care, acute care, ambulatory care or home health and hospice care
• This position requires that you obtain an NPI number within 30 days of starting in the position. The NPI process is free, and the application may take several weeks to process.
