Registered Nurse DP / SNF Full Time

- Position qualifies for $20k sign on bonus with 3+ years experience in acute care.
PATIENT POPULATION :
The patient population served in the DP / SNF is 21 years old and older. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.
Is responsible for the assessment, treatment or care appropriate to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patient’s needs.
Possesses knowledge of growth and development and has an understanding of the range of treatment needed for all patient groups
Must be able to work normal / scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call.
Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

LICENSURE / CERTIFICATIONS :
An individual must have and maintain current licensure as a Registered Nurse in the state of California. Must have and maintain current BLS certification or obtain within orientation process, prior to hands-on patient care.
Responsibilities and Essential Functions :
- Indicates Essential Function
- Is responsible for the assessment, treatment or care appropriate to the ages of the patients served, i.e., Adult (from 21 years of age), or Geriatric.
- Demonstrates the ability to obtain and interpret information in terms of the patient’s needs; possesses knowledge of growth and development;
and has an understanding of the range of treatment needed for these patient groups. Reviews / revises / records an individualized patient history and assessment and utliizes holistic assessment skills, recognizing subtle, covert or abnormal signs and symptoms that incorporates each of the following : a.
Involvement of patient / significant other.
b. Discharge needs.
c. Psychosocial needs.
d. Biophysical needs.
e. Environmental needs.
f. Teaching needs.
Demonstrates problem identification skills based on current physical / behavioral science knowledge base.
a. Identifies nursing diagnosis and interdependent nursing problems and selects applicable nursing care standards to guide patient care.
3 Care plan reflects :
a. Recognition of issues / concerns that could impact development of mutual goals.
b. Utilizes interdependent nursing standards.
c. The care plan is specific and unique to the individual patient.
d. Discharge needs of complex / multi-system patients.
e. Routinely reviews / revises the plan of care based on changing patient status, progress toward established goals, and discharge readiness needs.
4* According to established clinical policy, procedure and standards :
a. Safely performs nursing care including safe administration of medication.
b. Demonstrates an understanding of the use, actions and interactions of drugs used in combination for patients with multi-system problems.
c. Troubleshoots equipment.
d. Demonstrates skill in clinical problem solving.
e. Implements interventions when the patient is at covert risk of injury or complication.
5 The effectiveness of the plan of care is evaluated and communicated through report; recommendations are made for future care and is consistent with current documentation.
6 Evaluates effectiveness of nursing care by :
a. Reviewing / revising the plan of care.
b. Makes recommendations to health care team members.
7 Communicates evaluation of, or changes in, patient status providing supportive data to the physician or health care team as appropriate.
8* Evaluates teaching effectiveness based on patient response and revises teaching plan to accommodate patient / family comprehension.
a. Liaison activity between the patient / family and members of the health care team.
9* Demonstrates adherence to "National Patient Safety Goals for Hospitals" as defined by scope of practice.
a. Documentation reflects compliance with established hospital policies and procedures.
b. Demonstrates skill and ability to perform waived testing as per established policy and procedure.
c. Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice.
d. Engages the patient through teaching in activities that support the NPSGs.
10 Performs general and unit-specific skills independent of supervision.
11* Adheres to OSHA Guidelines / Universal Precautions as described in hospital policy.
12 Assists in the orientation of new staff.
13* Demonstrates proper infection prevention skills to include standardized protocols or procedures related to central lines and indwelling Foley catheters.
14 Assumes or supports charge nurse role and makes patient care assignments based on patient acuity, staff qualifications and available resources.
a. Organizes and establishes patient care priorities based on changing patient and / or unit status in a cost-effective manner.
b. Delegates appropriate tasks to unlicensed staff and ensures their completion.
15* Consistent with unit's quality assessment plan :
a. Conducts monitors and articulates outcomes to staff and management.
b. Makes recommendations for improvement in patient care.
c. Assists in formulating quarterly report.
16 Reports quality or risk management issues through appropriate lines of authority, i.e. administrative or clinical.
17 Demonstrates adaptability / flexibility in meeting unit needs.
a. Provides staffing support as requested by Director, i.e. scheduling flexibility, working extra shifts on short notice, low census days.
b. Demonstrates ability / flexibility to assist in unusual clinical situations, i.e. temporary reassignment, changes in patient activity / reassignment, unscheduled admissions, and life threatening situations.
c. Promotes an atmosphere to open communication with respect for the individual.
d. Accepts and responds to changes in a positive fashion.
e. Interpersonal relationships demonstrate an understanding of personal responsibility to represent nursing and Sierra View Medical Center in a courteous, helpful and productive manner.
f. Participates in quality assurance activities and integrates findings into practice.
g. Demonstrates sensitivity and awareness of patient's rights in handling confidential patient information.
18* Represents appropriate practice and directs others to follow or reference hospital policies, procedures or standards of nursing care.
19* Models appropriate communication and assists other staff in the identification of appropriate communication channels and methods.
20* Serves as a clinical resource and provides direction in patient care when working with aides, LVNs, students and peers.
21 Actively participates in unit problem-solving and communication by involvement in staff / unit rep meeting.
22* Models and articulates the role of professional nursing to other members of the health care team.
a. Maintains current knowledge of health care trends that affect the clinical practice of nursing on the unit.
b. Upholds all aspects of the Nursing Practice Act as defined by the California Board of Registered Nursing.
c. Demonstrates standards of documentation as evidenced by individual chart review and no letters of education in file.
d. Provides evidence of adherence to all nine ANA Code of Ethics for the nursing profession.
23 Knowledge of long term care regulations.
24 Possesses assessment skills to include respiratory assessment.
25 Possesses ability to initiate, monitor and discontinue all intravenous fluids including total parenteral nutrition and chemotherapy and possesses the knowledge and skills to properly care for central line including porta-caths and triple lumen catheters.
26 Promptly notifies the attending physician or designee of changes in patient condition or of significantly abnormal lab values.
27 Knowledgeable of Resident Rights and does not infringe on those rights.
