Behavioral Health Connections (BHC) in Phoenix is seeking a dynamic and talented Referral Management Clinician - Full Time that serves to assess and coordinate the care of patients referred for Mental Health Services.
They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs.
This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.
Job Duties / Responsibilities :
- Review assessment referrals received from various sources in collaboration witht he RN and other clinicians to assess patient needs.
- Assist in coordinating patient care and communicating with referring.
- Maintain patient records, updating databases, and ensuring accurate documentation of patent health information.

Qualifications
EDUCATION
Master’s degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
LICENSURE / CERTIFICATION
Preferred Current license in the State of Arizona as a Licensed Professional Counselor ( LPC ), Licensed Master Social Worker ( LMSW ), Licensed Clinical Social Worker ( LCSW ). RN or LPN
Current CPR, Vaccines, if required by a customer hospital. ( Customer hospital means the hospital to which BHC is contracted to provide mobile assessment services.)
EXPERIENCE
Preferred 1-year work experience working with Assessment and Referral department in a Behavioral Health Setting.
Completion of appropriate internship, including the provision of individual, group or family therapy / counseling.
Experience providing individual, group and family therapy / counseling.
Mobile assessment experience preferred
