Position Description : SUMMARY OF POSITION :
Provide case assistance services to older clients and families in accord with the standards of the social work profession and requirements of the funding source to those 60 years and older residing in Spring Creek Towers allowing them to successfully age in place.
DUTIES AND RESPONSIBILITIES :
Service Coordinator links tenants within the project to supportive services or medical services provided by public agencies or private practitioners within the general community.
The Service Coordinator may assess service needs and determine eligibility for public services. The major functions of the service coordinator are :
Provides general case management (including intake) and referral services to all residents needing such assistance.
Establishes linkages with all agencies and service providers in the community; shops around to determine / develop the best "deals" in service pricing to assure individualized, flexible and creative services for the involved residents).
Refers and links the residents of the project to service providers in the general community; these are, for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening / wellness and legal advocacy.
Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.
Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.
Helps the residents build informal support networks with other residents, family and friends.
May provide training to project residents in the obligations of tenancy or coordinate such training.
Participates in in-service trainings and staff meetings.
Ensure accuracy and timeliness of case records, reports and statistical information.
Performs such other duties as assigned.
QUALIFICATIONS :
Graduate of an Accredited College or University (BA or BS) in human services or other related field
Demonstrated organizational skills
Experience working with Senior preferred.
Ability to visit clients in their homes.
Understanding and appreciation of the role of race, religion, ethnicity, sexual preference and individual values as it relates to serving clients and their families.
WORK ENVIRONMENT
This job operates in a professional office environment, periodically at nearby community center and other locations within the Spring Creek Towers complex.
Occasional meetings in other boroughs of NYC
This is a busy office and work can be crisis oriented and unpredictable; many clients and families have multiple and complicated situations
Assessment and services provided to homebound seniors in their homes if needed
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines, etc.
PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to stand;
walk; use hands to finger, handle or feel and reach with hands and arms. The employee must frequently lift, carry and / or move objects up to 10 pounds and occasionally lift, carry, and / or move objects up to 25 pounds.
The employee must frequently be able to walk up and down approximately 7 stairs in the office as well as walk from the office to apartments or other locations around the housing complex when conducting home visits or other activities.
Meetings in other boroughs or locations outside the complex may require use of public transportation.
TIME COMMITMENTS
Hours are generally Monday through Friday from 9 : 00am to 5 : 00pm. Must be available for special events and trainings.
