Emergency Department Case Manager (RN) needed for a night-shift contract opportunity with Yoh’s client located in Palo Alto, CA!
Top Skills Should You Possess :
- Acute Care or Emergency Department Nursing Experience
- Recent Case Management Experience
- Ability to work M-F, 2 : 30 PM-1 : 00 AM with rotating weekends (Sunday-Saturday)
What You’ll Be Doing :
- Responsible for coordinating continuum of care and discharge planning activities for a caseload of assigned patients.
- Coordinating all facets of a patient's admission / discharge.
- Performing utilization review activities, including review of patient charts for timeliness of services as well as appropriate utilization of services.
- Ensuring optimum use of resources, service delivery, and compliance with external review agencies' requirements.
- Act as consultants to the clinical team, service lines, and other departments regarding patient assessment and patient care and participate in program development and quality improvement initiatives.
- Apply guidelines and collaborating with multidisciplinary teams, influence and direct the delivery and quality of patient care.

What You Need to Bring to the Table :
- Active and unrestricted California RN license
- Bachelor’s Degree in Nursing preferred
- Current BLS certification
- Two (2) years of progressively responsible and directly-related work experience
- Previous Case Management experience, preferably in an Acute Care and Emergency Department Nursing Experience setting
- Excellent verbal and written communication skills
- Excellent computer skills
