Your Role :
Schedule : Monday through Friday, full-time days
Key Responsibilities :
Align all activities with the vision of Elara Caring’s leadership.
Assess hospice patients and families to identify their needs.
Manage a caseload, including planning, coordinating, and evaluating treatment plans.
Conduct initial and follow-up patient evaluations.
Communicate significant findings and changes to the Clinical Supervisor and other relevant personnel.
Revise care plans as needed and provide counseling and instruction to patients and families.
Teach hospice philosophy and pain management to staff in various care settings.
Supervise home health aides and Licensed Practical Nurses / Licensed Vocational Nurses.
Provide support at the time of death and perform bereavement assessments.
Document medication regimens and maintain confidentiality in compliance with HIPAA.

Bachelor’s degree required, with additional one year paid professional experience, or one year as a Registered Nurse or Licensed Practical Nurse in lieu of an Bachelor’s degree.
