The RN Case Manager is responsible for supervising and evaluating skilled nursing services in accordance with the Physician Plan of Care, participates in activities relevant to the direct provision of home nursing services, and is responsible for compliance with CHAP and Independence Plus policies and procedures, Federal and State Regulations, IDPA, DSCC and DCFS rules and regulations, and payer and accrediting standards.
This position reports directly to the Director of Clinical Services.

- Reviews patient clinical diagnosis, medications, procedures, and clinical course while establishing long term therapeutic patient goals
- Assures that the Physician Plan of Care accurately and appropriately prescribes nursing actions and is continually evaluated, updated, and fulfilled
- Coordinates adjunct therapies (referral and case documentation) including physical, occupational and speech therapists, medical social workers, home health aides and community resources and promptly updates and communicates Plan of Care changes to field employees and adjunct therapists
- Provides employee and patient / family educational in-services, orientations, and announcements
- Participates in program evaluation (e.g., Performance Improvement, Safety / Risk Management, Ethics Committee and Professional Advisory Group), licensure and accreditation surveys, and implements plans of correction and monitors compliance as required
QUALIFICATIONS :
- Current license as a Registered Nurse in the State of Illinois
- Graduate of an accredited school of nursing, baccalaureate degree preferred
- Minimum of one (1) year of recent nursing experience with Home Health, Private Duty, and supervisory nursing experience preferred
- Prolonged or considerable walking, standing, and driving; ability to lift fifty (50) pounds; ability to lift, position, and transfer patients;
ability to lift supplies and equipment; considerable reaching, stooping, bending, kneeling, and crouching; visual acuity and hearing to perform required nursing skills
