As a key member of the Population Health Team, the Social Worker, MSW, will contribute to results as defined by the principles of value-based care.

The position will collaborate with, and serve as an expert consultant to the Population Health RNs, providers, and practice staff to engage in management approaches, which optimize desired health outcomes.

Responsibilities will include acting as an advocate for the patient (beneficiary, member), family and / or significant others, acting as a liaison between providers, patients, care team members, skilled nursing facilities, home health, hospital staff and community partners / resources.

Acts as a resource to assist patients in need of community-based program assistance and empowers the patient with the skills to navigate the process for obtaining appropriate medical care and community assistance.

Telehealth visits for Community Based / Palliative Care Program

2 years' experience in direct patient care and / or case management.

Preferred : Experience working with the frail or elderly population. Knowledge of Medicare, Medicaid and CMS regulations.
