Pay Range : $39.16 - $58.74 / hour
Additional components of compensation may include :
- Evening & night shift differential
- Overtime
- On-call pay
Per Diem Staff will not be eligible to participate in any associate benefit plans sponsored by the Health System.

POSITION SUMMARY :

We are seeking a Physical Therapist to :
- Join our world-class team of doctors, nurses and other health care professionals in providing everything from preventive health care to highly specialized programs.
- Provide quality care in a compassionate patient- and family- centered environment.
- Plan, evaluate and administer physical therapy services and care to patients per physician's orders in their home on an intermittent basis according to the policies of the Agency.
- Direct physical therapy assistants in providing treatments to clients following Agency guidelines.
- Participate in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives.
AGE SPECIFIC CARE :
Provides age and culturally appropriate assessment / screening, interpretation of clinical and laboratory data and develops and implements age appropriate interventions and plans of care including education within the parameters of his / her position responsibilities and licensure.
EDUCATION / TRAINING :
Graduate of an accredited Physical Therapy program.
LICENSURE / CERTIFICATION :
- Current license from the State of Illinois.
- An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment.
- An applicant who fails to proved this documentation will not be hired.
- A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
- Bi-annual CPR certification required.
- Valid state driver's license and proof of liability insurance.
EXPERIENCE / SKILLS / ABILITIES :
- Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner.
- Willingness to accept responsibility.
- Ability to direct ancillary staff and organize work assignments.
- Leadership skills essential to the position.
- Knowledge of JCAHO, CMS, IDPH rules and regulations preferred.
- Successful completion of annual clinical, age and job specific competencies and skill verification tools required.
