About the Role :
As a Care Navigator, you will play a crucial role in fostering the well-being and quality of life of our residents. You will conduct assessments, plan and facilitate support, and collaborate with cross-functional teams to ensure our residents thrive safely in our community.

Key Responsibilities :
Connect and Support : Establish rapport with new residents and outline the wide range of services available within our community and from external partners.
Safety First : Ensure residents are familiar with safety measures, including pendants and sensor motion monitors.
Collaborate and Plan : Work with residents to assess, evaluate, plan, implement, and document needed resources and support.
Administrative Excellence : Ensure necessary paperwork and contacts are accurate and up to date.
Emotional Support : Serve as a backup to support Assisted Living residents with mental health or emotional well-being needs.
Team Collaboration : Work closely with cross-functional teams to support residents in thriving and remaining safe.
Resource Management : Evaluate options to balance cost and quality for optimal well-being and financial outcomes.
Counsel and Advise : Provide guidance on transition and end-of-life choices, ethical concerns, and legal questions.
Continuum of Care : Manage care with KPI metrics and create monthly reports.
Smooth Transitions : Support residents relocating between Independent Living and Assisted Living, ensuring a smooth transition.
Community Engagement : Collaborate with the Community Engagement Manager on relevant referrals and opportunities for shared community outreach.
Networking : Foster relationships with outside educational and resource entities related to aging, mental health, and allied services.
Qualifications :
Education : Bachelor’s or Master’s Degree in Social Work, Occupational Therapy, or RN from an accredited institution, or a minimum of five years’ experience as a Certified Geriatric Care Manager.
Experience : Minimum three years’ experience serving older adults in a community-based setting.
Skills : Understanding of older adults, dementia, aging issues, complex family dynamics, and transition plans. Familiarity with Assisted Living and interdisciplinary team approach is preferred.
Proficiency in Windows-based computer systems and Microsoft Office products.
