JOB TITLE : Housing Case Manager
FLSA : Non-Exempt
REPORTS TO : Program Director
LOCATION : Yonkers, NY
SALARY : $45000.00 - $47000.00
SUMMARY : Provide housing-focused case management services to homeless families in an emergency sanctuary family shelter to assess families for housing options and secure permanent housing.
Ability to speak, read and write in Spanish is required for this position.
DUTIES & RESPONSIBILITIES :
- Formulates a housing plan with residents to ensure a timely transition to permanent housing and appropriate usage of housing subsidies and special programs that may be available for migrant families.
- Facilitates individual and group tenant education to provide families with information, resources and skills to help them secure and maintain permanent housing.
Topics include interviewing skills, understanding leases, landlord / tenant rights and responsibilities, financial literacy, and daily living skills.
- Refers residents to specific housing opportunities.
- Provides crisis intervention services as needed.
- Participates in case conferences.
- Assists residents with completion and filing of housing and rental subsidy applications.
- Helps residents secure moving costs from DHS, including broker’s fees, security deposits, first month’s rent, furniture allowances and arrange utilities and utility payments.
- Links families with community-based providers that are able to meet their long-term social support and service needs.
- Maintains accurate written record of services provided to families and all required documentation in CARES.
- Maintains demographic and other needed information regarding residents
- Submits accurate, timely reports as requested.
- Other duties as assigned.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS :
Bachelor Degree in Social Work or Human Services Field preferred. At least 1 year of case management experience working with the homeless, mentally ill, substance abuse and / or low-income populations.
Bilingual required. Strong relationship building skills required. Ability to work independently but know when to raise issues to supervisor.
Excellent organizational and time management skills necessary. Ability to effectively multi-task and follow-up on a timely basis.
Strong PC skills required including Word and Excel.
