THE POSITION :
Community and Connection Cases Close to Home
Hours : Various hours that include days, evenings, nights and weekends. Full time, part time and PRN shifts available.
Case Details : Various ages of children ages birth thru 30+years old with technologies that might include g-tube, trach and ventilator.

Responsibilities
Assist in development, review, and revision of the plan of care.
Provide nursing care according to physician's treatment plan and client's individual plan of care.
Observe, record, and report reactions to treatment and any changes in client's condition.
Provide ongoing instruction and supervision to client, family, or other health team personnel.
Monitor and document patient's condition and provided care services through electronic charting.
Participate in education programs, quality assessment activities, and staff meetings.
Requirements
Active Licensed Practical Nurse or Registered Nurse License
Problem solving skills and ability to multi-task
Excellent Communication Skills
Positive, Professional attitude
