About the Role :
We are seeking a highly motivated and experienced Family and Health Coordinator to join our team in Palmetto, FL. As the Family and Health Coordinator, you will be responsible for overseeing and coordinating the delivery of family and health services to our clients.
You will work closely with other professionals to ensure that our clients receive the best possible care and support. Your main goal will be to improve the health and well-being of our clients and their families.
Salary Range : $21.00 - $26.00 (based on experience)
Minimum Qualifications :
- Bachelor's degree in social work, nursing, or a related field
- Experience working in a family and health services setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational and time management skills
Preferred Qualifications :
- Master's degree in social work, nursing, or a related field
- Experience working with diverse populations
- Bilingual in English and Spanish
- Experience with electronic health records
- Certification in a related field, such as Certified Family Life Educator (CFLE)
Responsibilities :
- Develop and implement family and health programs that meet the needs of our clients
- Coordinate and monitor the delivery of services to ensure that they are of high quality and meet the needs of our clients
- Collaborate with other professionals, such as social workers, nurses, and doctors, to provide comprehensive care to our clients
- Maintain accurate and up-to-date records of client care and progress
- Provide education and support to clients and their families to help them manage their health and well-being
