Education, Experience, and Licensure Requirements :
- Licensed Physical Therapist (PT) in the state of Pennsylvania.
- CPR certification (BLS) preferred.
- Experience in sub-acute rehab and long-term care settings preferred.
- Strong interpersonal and communication skills.
- Ability to work effectively in a team-oriented environment.
- Committed to providing high-quality care and improving patient outcomes.
- Strong knowledge of medical terminology.
General Statement of Responsibilities :
- Evaluate patients' conditions and create personalized treatment plans
- Implement therapeutic exercises and modalities to improve patients' physical function
- Educate patients and their families on at-home exercises and self-care techniques
- Monitor and document patients' progress throughout the treatment process
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
