Manages the daily operations of case management department and case management staff. Ensures staff is informed of changes impacting their work by communicating updates, policy revision, performance outcomes and significant events.
Evaluates case managers’ practice for quality, timelines and process consistency.
Job Responsibility
Provides leadership to team members, supervisors, and managers by communicating and guiding toward achieving department objectives with either small portfolios at multiple sites or a single, larger portfolio Develops, communicates, and builds consensus for goals in alignment with system.
Selects, develops, manages and evaluates direct reports; and oversees the selection, development, management and evaluation of indirect reports.
Revises operational processes, policies and procedures to ensure overall program effectiveness. Develops staff performance evaluations in collaboration with Director of case management.
Participates in the evaluation of case management department activities. Ensures staff provides case management within scope of practice in compliance with department protocols.
Coordinates staff orientation and training programs; documents and assesses effectiveness of staff training. Develops Case Management reports to report departmental outcomes, trends and issues.
Collaborates with medical providers, the Director and Nursing Leadership, on program interpretation and guidance. Monitors individual staff caseloads and beneficiary treatment plans to ensure effective delivery of care, productivity and timeliness.

Job Qualification
- Master's Degree in Nursing or related field required, or equivalent combination of education and related experience. Bachelors or Masters must be in Nursing.
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
- 6-10 years of relevant experience and 5-7 years of leadership / management experience, required.
