We are seeking an in-office Clinical Intake and Patient Care Coordinator to carry out daily intake activities of Hospice agencies.
LVN or RN licenses preferred. Hospice Intake experience and Spanish speaking highly desired. This position also offers performance incentives.
Responsibilities :
- Handle DON assigned duties in a timely manner
- Assist in the process of patient intake and care coordination
- Maintain relationships with Case Managers, Nursing Homes, Hospitals, Physicians, and other referral sources.
- Assist with scheduling ambulance transportation, ordering supplies and equipment, bedside meetings
- Communicate with hospitals, foundations, nursing and board and care homes, and assist with Explanation of Hospice Benefits to patients and families
- Facilitate in-person or phone appointments with patients' families and physician
Qualifications :
- Must be a licensed LVN or RN
- Previous experience in healthcare administration or other related fields. Hospice experience preferred
- Familiarity with patient intake process
- Ability to work with a variety of healthcare professionals and administrator
- Strong organizational skills
- Strong communication and follow-up skills
