SUMMARY
Family Service League is seeking a part-time Social Worker for the Caregivers Support program in Commack. The Caregivers Support program provides individual counseling, facilitates support groups, and schedules educational training for people caring for a person over the age of 60 unless there is a diagnosis of dementia throughout Suffolk County.
RESPONSIBILITIES
- The Social Worker will conduct intakes, assessments, and treatment planning.
- Provide individual counseling.
- The Social Worker will provide off-site support group facilitation.
- Coordinate educational programs for caregivers on topics, such as long-term care planning, stress management, and resources for caregivers.
- The Social Worker will provide information and referrals related to accessing health care, Medicare benefits, and nutrition programs.
- Document client contacts in electronic health record.
- Conduct home visits as needed.
- All other duties as assigned.
QUALIFICATIONS
Master’s Degree in Social Work, Mental Health Counseling, or a related field required.
Minimum of 3 years of experience working with seniors and their families, facilitating support groups, and providing support and information to those caring for someone over 60 years of age required.
Excellent interpersonal, verbal and written communication skills required.
Proficient computer skills, including Microsoft Office required.
Familiarity with electronic health records preferred.
Bilingual in Spanish preferred.
