Transportation Coordinator Compensation

  • The pay for this position is $50-$57 per hour
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Transportation Coordinator Highlights

  • The required availability for this position is full time
  • Monday through Friday, 8 am 5 pm

Transportation Coordinator Responsibilities

  • Obtain requests for transportation from the Transportation Vendor.
  • Obtain Physician Certification Statement (PCS) forms required for Non-Emergency Medical Transportation (NEMT) transportation from providers before the transportation occurs.
  • Consultation with AAH staff about coordination of the transportation request as needed.
  • Communication with the transportation vendor about the PCS forms and the transportation to be provided.
  • Document all processes in the tracking system, ensuring that all steps are followed.
  • Establish and maintain effective, ongoing relationships by facilitating communication and coordination of transportation with members, their caregivers, PCPs / Providers, Alliance departments, and the transportation vendor, as well as other identified resources to which the patient was referred, based on each member's continued needs.
  • Assist staff in identifying and providing outreach, orientation, and basic care coordination of transportation services to members that may benefit from services.
  • Establish and maintain effective, ongoing relationships by facilitating communication and coordination with members, their caregivers, PCPs / Providers, and the transportation vendor, as well as other identified resources to which the patient was referred, based on each member's continued needs.
  • Participate in meetings and training with other departments to provide care coordination of transportation.
  • Provide guidance, support, education, coordination of care and other assistance to members, and / or their family members, as they move through the healthcare continuum.
  • Provide telephonic or email support to other Alliance departments and members in the case and disease management programs to use the transportation benefit to meet their treatment / care plan goals in coordination with case managers as appropriate.
  • Demonstrate a comprehensive understanding of coverage and benefits to promote appropriate service utilization and increase member knowledge and satisfaction.
  • Recognize and resolve transportation issues or other problem areas promptly.
  • Educate and answer inquiries from members and / or their family members about benefits, services, eligibility, and referrals with a positive and professional approach, promoting participant, patient, and member satisfaction and retention.
  • Escalate transportation problems and patterns to leadership as appropriate.
  • Collaborate in a positive interdisciplinary approach with other CM / DM staff, Utilization Management, Medical Services, Provider Services, Member Services departments as well as community resources to ensure most appropriate level of care and optimal outcomes.
  • Maintain security, confidentiality, and integrity of electronic database.
  • If appropriate, work with state and federal eligibility and enrollment staff / vendors to assist in continuity in enrollment.
  • Provide cross coverage for other CM support staff.
  • Research and compile data for special projects.
  • Complete other duties and special projects as assigned.

Transportation Coordinator Requirements

  • High School Diploma or GED
  • Attendance is mandatory for the first 90 days
  • Bachelor's degree in health care related area of study, or Two years of college, with a minimum of two years’ experience health care
  • Have a cleared TB test prior to or within seven days of hire.

Transportation Coordinator Preferred Qualifications

  • Two years office experience in a medically related setting (e.g., physician office, home health department, case management, social work).
  • Two years of direct customer service experience in the healthcare field, preferably in a health plan setting and a working knowledge of medical and insurance terminology, care delivery and coordination.
