Job Summary :
The Addiction Medicine Provider (Nurse Practitioner or Physician Assistant) will join other providers, nurses, and social work staff in the Dimock Addiction Medicine Program.
This program focuses on patient-focused, cutting edge, evidence-based, and holistic care for patients across the substance use and recovery spectrum.
This position will provide comprehensive, patient-centered, trauma-informed medical assessment and support to patients with substance use disorders in both our Office Based Addiction Treatment (OBAT) clinic and Acute Treatment Stabilization (ATS) unit.
At the ATS, the provider will meet with patients seeking acute detox at time of admission to assess severity of withdrawal, initiate appropriate treatment protocols, review past medical history and home medications, and address any acute medical and psychiatric concerns.
The individual in this position will collaborate closely with a multidisciplinary team at ATS including case managers, aftercare planners, and behavioral health specialists to ensure high quality wrap-around care is provided to each patient and a safe discharge plan arranged.
At the OBAT clinic, within the Dimock Community Health Center, the provider will meet patients for induction onto medication assisted treatment, collaborate with nurse care managers on clinical maintenance, and support low-barrier wrap-around urgent care and primary care bridge services.
Essential Functions :
- Performs comprehensive medical assessments for Acute Treatment Services patients, including obtaining patient histories, performing physical examinations, ordering appropriate diagnostics and therapeutics, interpreting patient data, triaging to higher level of care if appropriate
- Monitors each patient’s response to medications and other therapeutics over their ATS stay or in MAT and MOUD treatment and revises the care plan as appropriate
- Supports engagement in the health care system by evaluating, diagnosing, and treating urgent care needs and primary care bridge services for patients unconnected to care while receiving treatment at OBAT.
This may include typical urgent care evaluation and management or support of longer-term critical chronic disease management.
- Provides respectful, patient-centered, trauma-informed medical care that is at the same time thorough and efficient
- Documents patient encounters meticulously and efficiently
- Collaborates and communicates effectively with other team members at ATS and OBAT, including clinical leadership, other APPs, case managers, aftercare planners, behavioral health specialists, and admissions staff
- Assists with the development and evaluation of practice guidelines and protocols
- Participates in quality and process improvement activities to improve patient care and / or related systems
Schedule :
- Seeking Full-Time position with some shifts at ATS and some at OBAT with administrative sessions as well.
- OBAT clinic is open Mon-Fri 8 : 30-4 : 30. ATS shifts are 8 : 30-4 : 30 Sun-Saturday
- Some weekend and holiday coverage of ATS may be required. Weekly overnight on-call coverage, 5pm 8am.
- ATS On-call coverage and periodic weekend coverage is shared by all full-time and per-diem Addiction Medicine provider staff.
- There is flexibility of scheduling available
Compensation :
$110,000 - $135,000 per year, commensurate upon experience, plus benefits
Required Education and Experience
- Certified Nurse Practitioner or Physician Assistant
- 2 year of experience in Adult Internal Medicine
- 2 year of experience working with vulnerable populations
- Prior experience in addiction treatment preferred
- Understanding of harm reduction principles and practices
- BLS Certification within 90 days of hire
- Valid Prescriptive Authority from the MA DPH-DCP and Federal DEA
- Fully vaccinated against COVID-19
